Healthcare workers at the Community Health Centre (CHC) in Palvancha, a tribal region of Bhadradri Kothagudem district in Telangana, successfully conducted a rare and high-risk normal delivery of twin babies, both presenting in breech position.

Twin pregnancies are typically considered high-risk, and the situation becomes even more critical when both babies are in breech, a condition where the babies are positioned feet- or buttocks-first in the womb instead of the normal head-down position. In most private healthcare facilities, such cases are immediately routed for a caesarean section due to the potential complications. But the team at CHC Palvancha defied the odds, proving that with experience, preparation, and coordination, even the most complex deliveries can be managed naturally, said Dr. Ravi Babu, district health officer.

The patient, a native of Palvancha mandal and a regular visitor of government hospitals, arrived in active labour. Upon examination and ultrasound, the medical staff identified that she was carrying twins, both in breech. Under the leadership of midwife Sujatha and with the support of trained staff and doctors, the team decided to attempt a normal delivery, having prepared the mother extensively in advance with exercises and counselling.

“We knew it was a twin pregnancy and that both babies were in breech. Normally, this would be an automatic caesarean case, especially in private setups. But our experienced midwives, with the support and supervision of doctors, managed a successful normal delivery,” said Dr. Ravi Babu.

The two male babies, each weighing 1.9 kg, were delivered safely. Both newborns and their mother are now stable and recovering well. Medical procedures at government hospitals are offered free of cost.. “In a private hospital, this would have cost at least ₹2 lakh.
